Membership Contract

  • Mountain View Dental Reserves the right to interpret any program stipulations.
  • The annual membership fee must be paid in full prior to treatment.
  • Total payment for treatment done at an appointment is due at the time of service. If full payment is not received at the time of service fee discount will be voided.
  • No refunds will be given in the event the member terminates the plan prior to the end of the year.
  • Membership benefits are rot transferable, have no cash value, and may not be redeemed for cash.
  • This is NOT an insurance plan and is not subject to regulation by state department of insurance.
  • NO insurance claims will be filed for members on this plan.
  • This plan is for individuals only. Each additional family member must pay and be on their own plan.
  • Membership fee may be adjusted annually.
  • Missed appointment fees are ineligible for the membership discount.
  • *CBCT scan cost not included in an emergency exam radiology workup, only standard Xrays will be covered.
